Welcome to the Jerome Prairie Transition Center!
At JPTC, our mission is to support young adults, ages 18–21, as they take their next steps toward independence. We recognize that this stage of life is filled with important transitions, continuing education, building job skills, developing life skills, and finding one’s place in the community. Our role is to walk alongside students and families during this journey, helping each young adult discover their strengths and reach their goals.
We believe every student has the potential to live a fulfilling and meaningful life. Through individualized instruction, hands-on learning, and strong community connections, we work together to build confidence, self-advocacy, and the practical skills needed for adulthood.
